Temperature difference between your hand and the water. Heat always flows from a hotter body to a colder one. So you lose heat to the water till the temperature of the water and ur hand is the same. Thereby you feel your hand's cold.

Due to exchange of heat between the body and cold water when they are in contact. The body being at higher temperature transfers more heat to cold water than heat obtained from cold water resulting in a sense of coldness and thus we percieve it as cold.
