well i dont know the first part.....but i know second part
modulation index is the ratio of amplitudes of message signal and carrier wave..........from the graph the amplitude of message signal is 1 and that of carrier wave is given as 2.....modulation index == 1/2.........

for fig.see the book it is given there..
for equaation of the graph....
as clear from graph of modulating signal..... y=1 for x belongs to (k ,k+1/2) y=-1 for x belongs to (k +1/2,k+1)
k=0,1,2..... therefofre the equation of modulated wave will be.............
(1+2)sin(8*pi*t) for x belongs to (k ,k+1/2)
(-1+2)sin(8*pi*t) for x belongs to (k +1/2,k+1)
also see that the carrier wave is periodic with period 1/4 (period of carrier wave=2pi/8pi=1/4)
