Width will depend upon the residual charge on the cloud. Striking of lightening is like conduction of high current through your body, So after effects will be same as that of heavy shock

the radius of the luminous portion of a lightning strike is perhaps only
0.5 m long. but the radius of the actual strike may be upto 6m in radius

u cannot assume to be safe even if u are at a greater distance from the lightning strike point as the electrons dumped by the strike travel the ground. such ground currents may be lethal.

other effects of lightning may be paralysis and neurological disorders
