We do take it into consideration.This maay help:Cosider a string over a pulley.One end has mass8 kg. and the other has mass 12kg.Obv.the latter goes down.Now,we say 120-T=12a &T-80=8a,a being acceleration.So,we are considering the vector aspect of tension.
