answer for first is yes
consider a particle performing circular motion with constant accn, then the dir of its velo are opposite at any two diametrically opp pts on circle

a) consider a ball thrown vertically up... acc is const and downwards, but eventually the ball comes down (ie. vel reverses)

b) not necessarily.
consider STRAIGHT LINE MOTION........ lets say along +ve x direction, velocity & acceleration are 10 in magnitude initially.
now say that acceleration decreases from 10 to 2 constantly; in this time onterval too, velocity INCREASES in +ve x direction (as the acceleration even while and even after it reduces is still along +ve direction)
so here, direction of change in acceleration is -ve x (as it decreases from 10 to 2) and direction of change in velocity is still on +ve x direction.

hope the 2nd one is clear... if not, read slowly 1ce again

for first one, consider a ball thrown up . acc is always downwards whereas velocity is first upwards then downwards

for second one, consider projectile motion , g is downards all the time but direction of change of v keeps keep changing.
