Hi,
Firstly and most importantly, relax and hold on. Boost up your confidence with the fact that you have done some decent amount of preparation already.

Next, shortlist important topics in all the 3 subjects.
Maths: Coordinate, Calculus, Trigonometry, Permutations and Combinations, Probability.
Physics: Mechanics, Heat (simple to understand as well), Electricity and Magnetism (EM induction, etc.), Modern Physics (relatively simple and important).
Physical Chemistry: Mole Concept, concept of Equivalents, Equilibrium, Chemical Kinetics, Electrochemistry
Organic and Inorganic Chemstry: Read basic class XI and XII books.

Very Important: Solve last 5 - 6 years IIT-JEE question papers. This will help you in more than one way.
