in a carbonyl group the double bond between carbon and oxygen shifts in favour of oxygen as it is more electronegative than carbon.........this results in a positive charge on carbon and negative on oxygen.......and carbon with positive charge is unstable and it combines with nucleophile in the medium that is CN-..........and i suppose since oxygen is stable with its negative charge, nucleophile attacks first follwed by the addition of hydrogen to the oxygen
