A buffer solution is defined as that solution whose pH does not change with addition of any other chemical substance in small amounts....that's why buffer solutions are known as constant pH solutions If u take H+ conc to be (H+)buffer - x then ur buffer is not a buffer afterall H+ of solution remains constant for a buffer ,that's why x is not subtracted
